Clinical Data Exchange (Da Vinci CDEX)

Part of a set of Prior Authorization APIs allowing providers to transmit solicited or unsolicited documents to payers using FHIR.

Clinical Data Exchange (Da Vinci CDEX)

Payers frequently need additional clinical information to process claims, appeals, prior authorizations, and quality programs. Historically, this information is exchanged by fax, secure email, portals, or manual uploads—slowing down care and creating administrative overhead. CDex provides a FHIR-based mechanism for providers to send clinical documents or data directly to payers, either when requested (solicited) or proactively (unsolicited).

CDex focuses on simple, direct exchange patterns: sending clinical packets or structured documents without requiring complex workflow engines or custom integrations.

Provider & Care Team Experience

  • Send clinical attachments directly from the EHR or care management system.
  • Fulfill payer requests quickly using FHIR-based DocumentReference exchanges.
  • Attach clinical notes, summaries, imaging reports, or lab results.
  • Avoid delays caused by faxing or logging into multiple payer portals.

Technical & Standards Alignment

  • FHIR R4 Resources: DocumentReference, Bundle, QuestionnaireResponse, Communication, Provenance.
  • Solicited Mode: The payer requests specific clinical content; the provider sends the required documents or data.
  • Unsolicited Mode: The provider proactively sends supporting clinical information relevant to a claim or prior auth.
  • Supports narrative documents (PDF, notes) and discrete FHIR resources.
  • Flexible enough to support claim attachments, prior auth information, and supplemental documentation.

Governance & Compliance

  • Supports CMS expectations for reducing manual chart collection and improving electronic exchange.
  • FHIR-based exchange ensures provenance, versioning, and traceability.
  • Minimum Necessary enforced by scoping to exactly what is requested or required for treatment or payment.
  • Audit logging for requests, submissions, and acknowledgments.
  • Aligns with HIPAA TPO rules for payer–provider data exchange.

Value by Audience

Clinicians & Care Teams:

  • Rapid, structured way to send the required information.
  • Reduced time chasing down documentation requests.
  • Fewer payer callbacks or rejections due to missing attachments.

Payers:

  • Faster access to the clinical data needed for claims, audits, or prior authorizations.
  • Reduced manual processing of faxes and portals.
  • More complete and reliable data for adjudication and quality programs.

IT & Architecture:

  • Lightweight FHIR-first integration pattern for transmitting documents.
  • Works alongside PAS and other exchange workflows.
  • Requires minimal EHR build compared to complex Task workflows.

Capabilities

  • Send clinical documents (PDF, notes, summaries, images) using DocumentReference.
  • Transmit structured FHIR data elements when required by the payer.
  • Fulfill payer-initiated requests (solicited) or send proactive documentation (unsolicited).
  • Support for attachments tied to claims, prior auths, or quality reporting.
  • Provenance and metadata for tracking completeness and timeliness.

Da Vinci Alignment

CDex is defined in the Da Vinci Clinical Data Exchange (CDex) Implementation Guide, which outlines the solicited and unsolicited patterns, FHIR profiles for document exchange, and usage examples for prior authorization, claims, audits, and care coordination.

Optional Alternative Approaches

Organizations may choose simplified CDex patterns that focus on direct document transmission without Tasks or workflow automation. These models allow providers to send information efficiently while keeping implementation lightweight and aligned with real-world operations.
