Cross-Enterprise Patient Search

Real-time patient search and match across enterprise and external registries to prevent duplicates at the point of registration.

Cross-Enterprise Patient Search

The Cross-Enterprise Patient Search app empowers registration, HIM, and EMPI teams to search, match, and validate patient identities across internal EMRs, enterprise master patient indexes, and external registries in real time. At registration, where duplicates most commonly originate, it evaluates inbound demographics, issues parallel FHIR $match and PDQm queries, and surfaces near-matches with confidence scoring, alias chains, and demographic deltas.

Users are guided through link/deny workflows, including rationale capture, survivorship checks, and optional escalations to EMPI specialists. This prevents downstream duplicate records, mitigates merge risk, and improves longitudinal identity accuracy across care settings.

Why it matters

Identity fragmentation begins at registration. Once a duplicate is created, it spreads across billing, clinical systems, HIEs, payer APIs, and longitudinal clinical records. Stopping duplicates before they enter the ecosystem improves patient safety, cycle time, data quality, and interoperability success.

Benefits

  • Prevents duplicates at point of entry: Detects near-matches before new MRNs are created.
  • Improves match accuracy: Uses industry-standard FHIR $match and PDQm for federated identity lookups.
  • Strengthens governance: Link/deny decisions are logged with rationale and survivorship context.
  • Supports multi-facility workflows: Designed for systems with multiple ADT sources and facility identifiers.

Capabilities

  • Real-time parallel identity queries: FHIR $match + PDQm + internal EMPI lookups.
  • Weighted match scoring: Confidence ranking based on demographics, recency, and verification levels.
  • Link/deny with provenance: Captures user rationale and audits every resolution step.
  • Escalation to EMPI queue: Routes complex identity cases to HIM/EMPI specialists.
  • SMART on FHIR launch: Available inside EMR/registration contexts for embedded workflows.
  • Alias/merge guardrails: Highlights risky matches and prevents unsafe auto-creation or merges.

Great for

  • Registration & Access Teams: Prevent duplicate MRNs at first touch with guided match review and deny workflows.
  • HIM & EMPI Analysts: Validate identity matches and adjudicate borderline cases using enterprise match logic and audit trails.
  • Interoperability & Identity Architects: Align search and match processes across PIX, PDQm, $match, and HL7 v2 ADT flows.

Integration & Standards Alignment

  • FHIR R4 $match and Patient search
  • IHE PIX, PDQ, PDQm, and XCPD
  • Works with ADT-driven EMPI and cloud MPI platforms
  • FHIR AuditEvent and Provenance for traceability
  • HIPAA-aligned minimum necessary + audit readiness

Outcome Metrics

  • ↓ reduction in new duplicate MRNs
  • ↑ improvement in match accuracy at registration
  • ↓ drop in HIM merge workload

Interops Team delivers identity workflows built on Connectathon-tested PIX and PDQm profiles with FHIR $match orchestration, ensuring enterprise-grade identity integrity across HL7 v2 and FHIR ecosystems.
